32 year-old woman complied of a small swelling in the dorsum of her right hand and restriction of hand extension, without any tenderness or trauma. She carried a heavy box 2 week earlier. The lesion was elastic, and not attached to the surrounding tissue. It easily moved in accordance with third tendon motion (Video 1). With soft tissue tumor or ganglion prediagnosis it was surgically explored. An intratendinous ganglion was seen in the tendon of the third finger, with no inflammation. It was excised and the remaining of tendon was simply repaired. Two weeks later, hand movements were improved. No recurrence was observed for six months. Intratendinous extansor hand ganglion is a rather rare type of ganglia, with a few reported cases [1,2]. The etiology is controversial (tenosynovitis, degeneration, trauma) [2,3]. They are usually asymptomatic. Unlike conventional ganglia, it should not be ruptured. Restriction of extension of hand usually disappears after surgery [1-3].
